When you give birth

Submit a birth notification to the Civic Affairs Section at Urayasu City Hall within fourteen days of your child’s birth.

Where to submit

• Municipal office administering your current address
• Municipal office administering the place where your child was born
• Municipal office administering your registered domicile (only if one parent is a Japanese national)

What to bring

• Birth certificate (obtained at the hospital)
• Mother and Child Health Handbook
• Birth notification (obtained with the birth certificate at the hospital)

If you and your spouse

partner are both foreign nationals, please notify the embassy or a consulate of your home country. In this case, your child cannot obtain Japanese nationality even if they are born in Japan. Please notify the embassy or consulate of your home country of the childbirth.

Certificates available after submitting a birth notification

You can obtain a certificate of receipt or certificate of registered matters at the Civic Affairs Section (350yen per certificate).
